Scientific production on aneurysm of the abdominal aorta in SciELO in the period 2021-2023

Abstract

Introduction: an abdominal aortic aneurysm is a multifactorial disease in which genetic and environmental factors intervene. Its incidence makes it one of the pathologies of the cardiovascular system with the most research on it. SciELO as a collaboration program for the dissemination of scientific material stands out in its production on this disease. Objective: to characterize the scientific production on abdominal aortic aneurysm in SciELO in the period 2021-2023. Methods: bibliometric and descriptive study of scientific production on abdominal aortic aneurysm in SciELO in the period 2021-2023. In the search, 69 articles were rescued, which were analyzed considering the following variables: type of publication, year of publication, country of origin, language and bibliographic references. Graphs and tables were prepared to represent the data and a quantitative analysis was concluded. Results: the most frequent type of article was the case report with 46.38% and the year 2021 was the year with the most publication (44 publications). The country of origin with the highest number of articles was Brazil (27.54%); Spanish was the language most used in the publications (55%). The Price index obtained had a maximum value of 0.28. Conclusions: the scientific production on abdominal aortic aneurysm in SciELO presents a predominance of case reports. Brazil is the country that makes the greatest contributions to the platform on the topic and the Spanish language was the most used.

Introduction

An abdominal aortic aneurysm (AAA) is a localized dilation of the infrarenal aorta. It is a multifactorial disease involving genetic and environmental factors [1] .

The overall worldwide prevalence of AAA is 1.9 at 18.5 % in men and 0 to 4.2 % in women. Autopsy studies have observed an AAA frequency of 0.5 to 3.2 %, with an incidence of 4 to the 8th % in males between 65 and 80 years of age, and 1.3 % in females of similar age. [2]

The biochemical and pathophysiological characteristics of AAA are complex. and three stages are identified in its formation: development, growth and rupture . The most frequently associated risk factors in the literature are: smoking (considered the most important), male sex, age, family history of aneurysms, chronic obstructive pulmonary disease, hypercholesterolemia and presence of calcifications [2] .

The development of an AAA is a silent process whose natural evolution is rupture, which often has a fatal outcome [3] . Ruptured aortic-abdominal aneurysm is fatal in more than 80% of cases. % of cases. Risk factors include, in addition to those typical for AAA , age over 75 years, previous vascular disease, high blood pressure, and family history [4] .

AAA is a common disease associated with significant cardiovascular morbidity and mortality. There is currently controversy regarding the choice of treatment. Current evidence favors an interaction of the hemodynamic microenvironment with local and systemic immune responses [1] .

SciELO (Biblioteca Electrónica científica en Línea / Biblioteca Digital científica en Línea) is one of the most widely used academic databases in the scientific world. It is an online electronic scientific library that aims to disseminate scientific knowledge in developing countries, with an emphasis on Latin America and the Caribbean [5] .

The incidence and danger of this disease , along with its complications, make it one of the most important topics in the field of medical science research. For this reason, SciELO, as a support program for the open access research communication infrastructure, is one of the main sources on the topic. It contributes to the advancement of scientific research and also maximizes the visibility of indexed journals and, consequently, of their articles. Therefore, this research aims to characterize the scientific production on abdominal aortic aneurysm in SciELO during the period 2021–2023.

Method

observational , descriptive, bibliometric study was carried out on SciELO scientific publications from 2021 to 2023 on topics related to abdominal aortic aneurysm . The data were obtained through electronic consultation through the Internet to the SciELO website (http :// SciELO . org ) .

The website portal was accessed and a search was performed using the keyword Abdominal Aortic Aneurysm. The results were filtered and the publication years 2021, 2022, and 2023 were selected.

69 articles were obtained and downloaded in PDF format . Since this was a study in which the entire population could be included, no sampling techniques were applied . The inclusion criterion was: all articles published in journals indexed in SciELO during the study period. There were no exclusion criteria.

Variables taken into account were: type of article (original article, review article, case report, brief report, letter to the editor, special article); year of publication [2021], [2022], [2023] ; origin of the articles (Argentina, Brazil, Colombia, Cuba, Chili, Spain, Mexico, Peru, Portugal , Paraguay); language (Spanish, English, Portuguese) and bibliographic references (number of references and references less than five years from the date of publication of the article).

To determine the age of the references, the Price Index (PI) was calculated, which represents the proportion of the number of references less than five years old, with respect to the total number of references.

A database was created using the collected publications in Microsoft Excel 2010. Statistical analysis consisted of calculating absolute and relative frequencies.

Cuban ethical standards for health science research and the Second Declaration of Helsinki were respected. The study was conducted in compliance with the four basic ethical principles: respect for persons, beneficence, nonmaleficence, justice, and confidentiality .

Results

The highest percentage according to article type in the period studied is represented by case reports with 46.38 %, while the letter to the editor and the short report only represent 1.45 % each ( TABLE 1 ).

Brazil stands out in its scientific production on topics associated with abdominal aortic aneurysm, with a significant percentage of 27.54 % of the total articles on the topic during the period studied. Peru published the fewest articles, placing it last ( TABLE 2 ).

The articles were analyzed according to the language in which they were published in SciELO. It is notable that 55 % of the articles were published in Spanish, with a total of 38 ; while in Portuguese there was a lower representation, with only 19 % ( FIGURE 1 ) .

References to original articles from 2021 predominated (69.23 %). A total of 704 bibliographic references were used. The Price index showed a minimum of 0.18 and a maximum of 0.28 ( TABLE 3 ) .

Discussion

To achieve better results in the early diagnosis and care of patients with AAA, it is necessary to keep the topic up to date and scientific production increasing over the years [7] . The results of this study indicate that publications in SciELO about AAA in the period 2021-2023 are not consistent with what was stated, since during this period they have decreased significantly , a fact that leads to a poor update of the diagnosis and treatment of the disease.

Izaguirre Perez et al [8] , have pointed out that reporting a rare or unusual clinical case is seen by some professionals as the lowest level of evidence. These It provides preliminary information for future research leading to the practice of evidence-based medicine, but it is probably the oldest form of medical communication and the least practiced .

However , the highest percentage according to the type of articles was represented by case reports with 46.38 %, agrees with Medina Chiliquinga ER et al [9] . This result, despite being contradictory to what was stated in the previous text, It suggests that scientific production of this type, compared to other types of production related to cardiovascular conditions, helps to identify patterns, new advances and clinical experiences that enrich medical learning and training.

On the other hand, the letter to the editor and the brief report represent only 1.45 % , each one differs of those found by Estrada Araoz EG [10] . Nevertheless, the publication of a letter to the editor and responses between authors and readers favors the impact factor of a journal [11] . This result may be due to the complexity of the subject in question, which causes authors to feel little interest in publishing this type of article.

In this research, Brazil is the country of greatest origin of the articles with a significant percentage of 27.54 % of the total, a question that is confirmed , since an analysis in the 1996-2019 period this was the Latin American country with the highest scientific production [12] . Furthermore, in 2022 Brazilian journals represented 29.4 % of the total of these, at the same time, gathered 48.9 % of the documents and 50.1 % of articles indexed in SciELO and Redalyc [5] . Regarding articles of Peruvian origin , which turned out to be the lowest in number, with 1.45%, this coincides with another study by Beigel MF et al [13] ; it is one of the nations with the lowest contribution to scientific production with 1.92 %.

dazzling that the language with the lowest representation, with only 19% % was Portuguese, contrasting because it is the official language of the country of greatest origin . Another unexpected finding was that only 26 % of the English language is represented, as it is the universal and most prevalent language in medical scientific literature . This is consistent with Maz Machado 's research . A et al [14] , in which Portuguese and English represented the lowest amount with 1.94% .

The main indicator that expresses the aging of research is the Price index. This measures the percentage of documents referenced in a bibliography that are five years old or less [15] . According to the present results , in which the Price index ranges between 0.18 and 0.28 in the original articles, there is no agreement with Jiménez Franco et al [16] ; in their research this metric reached the figure of 0.63. This demonstrates the limited updating of bibliographic references on the subject in research .

Conclusions

Scientific production on AAA in SciELO shows a prevalence of case reports . Brazil is the country that contributes the most to the platform on the topic, and the least used language is Portuguese . The production of articles on the topic declined during the study period, as 2021 was the year with the highest number of publications.
