Synovitis, acne, pustulosis, hyperostosis, osteitis syndrome: clinical features and therapeutic approach

Abstract

Introduction: Synovitis, acne, pustulosis, hyperostosis, osteitis syndrome is an autoinflammatory and chronic disease. The estimated prevalence is one per 10,000 inhabitants and the most common age of presentation is between the third and fifth decade of life, with a slight predominance of the female sex. Due to the little knowledge that exists about this entity, scientific research is necessary to contribute to a better clinical-therapeutic approach. Objective: to describe the clinical and therapeutic characteristics of synovitis, acne, pustulosis, hyperostosis, osteitis syndrome. Methods: a bibliographic review was carried out from September to October 2024 from 23 references. Articles and publications from the Elsevier, Pubmed, SciELO databases, and the Google Scholar search engine were used. The descriptors used were: Acne; Hyperostosis; Osteitis; SAPHO syndrome; Synovitis and its English equivalents. Development: there are two well-differentiated clinical spectrums, the first characterized by combining cutaneous and joint symptoms while the second only affects the osteoarticular system. The initial treatment is based on symptomatic relief of the patient while maintenance therapy uses antirheumatic drugs, bisphosphanates and antibiotics. Conclusions: synovitis, acne, pustulosis, hyperostosis, osteitis syndrome is currently a diagnostic challenge. The association between joint and cutaneous symptoms guides the diagnosis, although the latter may be absent. The first therapeutic line is the association between non-steroidal inflammatory drugs and disease-modifying antirheumatic drugs with emphasis on methotrexate.
